Although "good" has ethical overtones, my assessment of whether a decision is good or not includes the following factors:
was the decision required to be made, specifically identified first?
were all the people who needed to be consulted about it, consulted before it was made?
were all the relevant facts and information considered when the decision was made?
were all irrelevant facts and information ignored when the decision was made?
were all the consequences considered when the decision was made?
were all the costs considered when the decision was made?
were all the alternatives considered when the decision was made?
did the decision include a provision for review at a set time?
were appropriate processes followed when making the decision?
is the decision consistent with your vision and values?
is it a real decision or just a failure to make a decision and accept the status quo?
was the decision made first and the justification obtained after ?
A decision is not "bad" simply because it turns out be a mistake, but could be bad if that decision was not properly made.
